Colorado Division of Water Resources forms are provided online for download in PDF format. Forms are no longer provided in MS Word .doc or .dot format to allow for direct completion due to security concerns, but fillable forms are being developed as an alternative.  Forms can be mailed or dropped of at one of our office locations.  Please make sure to include any required processing fees with your application.
